Our History
For more than four decades, Community Health Net has grown alongside the community it serves. Explore key milestones that have shaped our mission, services, and commitment to accessible care.
1985-1994
Community Health Net began in January 1985 as a small medical and dental clinic operating within the Booker T. Washington Center in Erie, Pennsylvania. Originally incorporated as Primary Health Care Services of Northwestern PA, the organization was founded to address unmet healthcare needs in the community.
On July 1, 1985, the organization was certified as a Federally Qualified Health Center (FQHC) under the Public Health Service Act—establishing a long-term commitment to accessible, community-based care.
During these early years, CHN began forming partnerships with local health organizations, laying the foundation for future growth and expanded services.
1995–2004
In the mid-1990s, Community Health Net entered a period of expansion and collaboration. In 1995, several community health centers—including the Hispanic American Council Metro Health Center, Hamot Medical Center, and Saint Vincent Health Center—were restructured under one unified organization. At this time, the name Community Health Net (CHN) was adopted to reflect the organization’s broader mission and vision.
Throughout this period, CHN expanded medical and dental services and established new satellite locations. In 1996, the organization purchased a downtown Erie building to support administrative and clinical operations.
By the early 2000s, CHN had added obstetrics and gynecology services and strengthened partnerships with Saint Vincent Health Center, Gannon University, and the City of Erie Housing Authority.
2005–2014
In 2005, CHN renamed its primary medical site the Daniel S. Snow, MD Health Center in recognition of the organization’s 20th anniversary.
Over the next decade, CHN expanded services across Erie County—opening dental clinics, partnering with behavioral health providers, and introducing specialty care programs to meet emerging community needs.
Key developments during this era included:
- Expansion of dental services across multiple locations
- Introduction of a 340B in-house pharmacy
- Launch of optometry and HIV/AIDS intervention services
These efforts strengthened CHN’s ability to provide comprehensive, integrated care.
2015–2019
Beginning in 2015, Community Health Net expanded into school-based healthcare, opening the Girard School-Based Health Center to provide age-appropriate care for children and adolescents.
In the years that followed, CHN integrated behavioral health services into its care model and expanded support for patients facing social barriers to health. Case management and care coordination services were added to help patients access resources beyond clinical care.
In 2019, CHN formally introduced Integrated Behavioral Health Services, further strengthening its whole-person approach to care.
2020–Present
In recent years, Community Health Net has continued to adapt and innovate. Services such as Medication-Assisted Treatment, healthcare for the homeless, and expanded case management reflect CHN’s commitment to addressing social determinants of health.
In 2022, CHN acquired additional property adjacent to its primary health center to support future growth. Telehealth initiatives—including the launch of Virtual Community Care—have extended CHN’s reach to patients in need.
Today, CHN remains accredited by the Joint Commission Ambulatory Healthcare Accreditation Program, reflecting its ongoing commitment to quality, safety, and excellence in care.
Looking ahead, CHN plans to expand services throughout Erie County and into rural communities—continuing its mission to improve quality of life through compassionate, professional healthcare.
