CHN participates with most local and national insurance plans. When you provide accurate insurance information, our billing team will submit claims directly to your carrier. Depending on your plan, you may be responsible for co-pays, co-insurance, or deductibles.

If you’re uninsured or underinsured, our Sliding Fee Discount Program helps reduce costs based on income and family size.

CHN’s Sliding Fee Discount Program makes healthcare affordable for everyone. Discounts are based on family size and household income, reducing fees by 20–100%.
